2008 Verification Summary Report

By November 15 of each year, all sponsors participating in the National School Lunch Program (NSLP) and/or School Breakfast Program (SBP) must have selected and verified a sample of applications approved for free and reduced-price meal benefits. (For details of the verification process, go to http://www.isbe.net/nutrition/htmls/forms_sbn.htm.)

By January 18 of each year, per federal regulations, all sponsors are required to submit a summary of these verification efforts electronically using the Verification Summary Report.

INSTRUCTIONS FOR ACCESSING THE VERIFICATION SUMMARY REPORT

STEP 1
Access the IWAS (ISBE Web Application Security) website and click on System Listing. Select Child Nutrition—ACES (Application and Claim Entry System) and click on Applications and Claims.

STEP 2
In the left-margin menu, ensure Search Type = Application; Application/Claim Type = Sponsor; Program Type = NSL; and Program Year = 2008, then click Search.

STEP 3
Using the drop-down box under Links, click on Verification Summary. Select the verification summary information at the district (sponsor) level or at the school (site) level. District level is best if you are a single site or already have numbers from your sites totaled. Site level is best if you have more than one school. Enter the numbers for each site and then consolidate into a district report (see Step 4). Click View and Submit Your Prepared Summary Report, and then proceed with entering the requested information.

If you have successfully submitted the verification summary, a message will appear stating Form Submitted Successfully. We recommend you print a copy of the report for your files. To do so, select Return to Main Menu and then View Your Submitted Summary Report. Click the printer icon in the upper right-hand corner of the screen.
