Based on the Verification Summary Report data submitted to ISBE, this file contains sponsors that have at least 1 site that is eligible (40% Identified Students or greater) or close to eligible (30-39% Identified Students) as indicated on the worksheet. If applying for CEO, the Identified Student numbers must come from April, 2013 and the worksheets below can be used to evaluate the implementation of CEO at your school or district.- SY2013-14 CEO Evaluation Worksheets– in order to determine which worksheet to use, the 60% rate refers to the district’s National School Lunch Program rate of reimbursement. Those districts claiming 60% or greater F/R lunches two years prior receive the 60% or greater rate of reimbursement. The Severe Need Breakfast, is a higher rate of reimbursement provided to sites that claimed 40% or greater of their lunches as F/R two years prior. This information can be found on your 2013 NSLP Approval Letter, which you may have a hard copy of or that can be located through the IWAS system in the Child Nutrition ACES program under document archive.
